DESCRIPTION:2016 Annual Banquet Award Nominations\n\nSubmissions will be accepted until close of business (5:00 p.m.) Friday\, October 14\, via mail\, email\, or fax or in person to the Chamber office.  Please review the attached criteria to ensure that your nomination is eligible for an award.  Click Here \n\n\n\nPlease sign and date the attached form and attach a letter explaining your reasoning for this nomination including the process that was completed to select the nominee (whether by committee vote\, recommendation by a board\, individual nomination\, etc.).  Please focus on ways that the nominee has given back to the community.  Include how the nominee meets the criteria on the attached page\, any known awards presented to the nominee\, significant dates\, milestones\, or contributions applicable to the nominee\, service projects or other significant acts done by or in conjunction with the nominee\, photos\, videos\, articles\, or other features that showcase the nominee\, and any additional information that might help the selection committee determine an award winner.  Project nominations must also include a letter from a sponsoring organization describing in detail what they consider the most significant aspect of the project.  Eligibility is based on the attached criteria for each award.  The deadline for submissions is 5:00 p.m. Friday\, October 14\, by mail\, fax\, or email or in person to the Chamber office.  No submissions will be accepted after this deadline.
